A. The information is always
stored on your home computer so you will always have your information at home.

A. No, there is more vital

carry these each day without a thought. Plus, other than an emergency page designed

Filling In and Updating your Portable Health Profile
Q. How do I get my Medical records from my
doctor and will there be a fee?
A. Your physician is
required by law to make available to you a copy of all of the information located in your
file/chart. Some physicians will charge a small fee, usually less than $25.00 for copying
expenses.

secured your records it should take no longer that 25-30 minutes to complete.

Q. How can I add medical images?
A. Secure your
medical images (EKG, CT Scan, MRI, X-ray, Mammogram) from your physician and have them
scanned as a JPEG image. This can easily be done on your home scanner or at any copy shop.
